All Veterans Memorial Brick Order/Donation Form Instructions
After reading the following instructions, click on the Link at the bottom of the page.
Step #1: (Donations Only) If you are making a donation to the Veteran's Memorial and not purchasing a memorial brick, please indicate on line #1 of the form. If you are purchasing a memorial brick proceed to step #2
Step #2: (Memorial Brick Purchase) $ 100.00 per BRICK Fill out the 6 lines of information, as you want it to appear on the memorial brick. Remember that a maximum of 20 characters per line is allowed. Character is defined as a letter, number or space. Optional symbols such as (American flag or cross) count as one line and are an additional cost of $ 15.00 / symbol.
Step #3: Fill out the section indicating who purchased the memorial brick.
Step #4: Make checks or money order payable to: Uniontown Community Park Association Step #5: If desired for your personal records make copies of all information being submitted to the address as shown below in step #7.
Step #6: Be sure to enclose your employer’s matching gift form if applicable.
Step #7: Print the "linked" form below and send it and the payment to the following address.
